Skilled Documentation for Licensed Nurses, DNS and Staff Development Professionals!  

Now that the focus on therapy volume has gone away with the new PDPM (patient driven payment model), will your daily skilled nurse notes support the skilled service provided? Per CMS, PDPM does not change skilled care requirements, however, we [CMS] expect provider risk will be more easily mitigated to the extent that reviews focus on more clearly defined aspects of payment such as documentation support for pt dx and assessment coding.

Will your daily nurse documentation support the SKILLED services provided and hold up in a Medicare review? Will your current nurse’s notes for skilled care identify and support the complexity of skilled nursing care provided?
In the past, therapy notes have been under a great deal of pressure to support the skilled services provided. With PDPM, the focus includes Skilled Nursing. Will your nurse’s notes support the residents need for “daily” nursing services?

In this class, Colleen will guide you through the Medicare skilled care criteria and expectation of supportive documentation. 

Current nursing documentation examples will be reviewed.

 

ABOUT OUR SPEAKER:  Colleen Marlatt RN, BSN, RAC-CT, RAC-CTA
Colleen, Founder and Owner of Cornerstone Clinical Consulting is passionate about excellent care for the aging population, loves the MDS and sharing guidance from the RAI (resident assessment instrument) MDS manual.  Colleen has been an active member of AANAC (American Association of Nurse Assessor Coordinators) since 2001.

Colleen has built her career on a foundation of acute & sub-acute direct patient care, staff supervision and training. Colleen has participated as an expert adviser to multi-disciplinary teams regarding (WA) State and Federal requirements for the MDS (Minimum Data Set); consulting and training on the RUGs III /IV categories for reimbursement. Colleen has worked with health care lobby groups to support advocacy for the aged and ongoing excellent healthcare as well as clarification to the MDS and RAI processes.

Colleen worked as a corporate clinical reimbursement specialist for 7 years. Colleen has been a MDS reimbursement consultant to long term care, Medicare and transitional care facilities in Washington, Idaho and California States. Major responsibilities have included: review of the MDS compliance process and systems to help assure accurate reimbursement for the care provided as well as review of clinical documentation and systems to assure the documentation supports entitled reimbursement. Colleen has guided many communities through successful Medicare and Medicaid appeal processes.

Colleen provides RAI manual focused education and in-services to LTC facility staff on accurate MDS coding criteria.

Outcome of the facility consultations has resulted in significant increases in entitled revenue to both Medicare and Medicaid.

Colleen was awarded the Leading Age 2016 Chair Award for her advocacy for ongoing excellence in elder care in our communities.
